Quality Star Ratings: Assigned by CMS from OASIS-based clinical outcome measures and Medicare claims. Half-star ratings (1.0–5.0 in 0.5 increments) reflect CMS methodology. Agencies below episode volume thresholds are unrated.

Patient Survey Stars: CMS Home Health CAHPS (HHCAHPS) program. Independently administered patient experience survey across professional care, communication, medication/pain/safety discussions, and overall agency rating.
