Top 25 home health acquisition targets per state, scored and ranked on quality, VBP performance, cost efficiency, and market position. Built for PE and REIT acquirers evaluating HHA portfolios.
Every agency in the state is scored 0–100. The report surfaces the top 25 targets with full data context. Tier thresholds are set at operationally meaningful thresholds — not arbitrary quartiles.
Four components weighted by acquisition relevance. Quality and VBP performance dominate because they determine regulatory standing and future Medicare reimbursement — the two biggest value drivers in HHA transactions.
The report is designed for the first stage of an acquisition workflow — identifying who to call before you start underwriting.

Structured for the acquisition workflow — market context first, then scored targets, then aggregate profile for portfolio-level benchmarking.
Both reports cover the same state. They answer different questions.

No. Agencies without cost report data receive a neutral score (50/100) on the cost efficiency component rather than being excluded. The score is still computed from the other three components. The Methodology section of each report notes which agencies had complete vs. partial cost data.
Scores are recomputed monthly as CMS Provider Data Catalog data refreshes. VBP scores update when CMS publishes new performance year data (typically annually). HCRIS cost reports update on a rolling basis as agencies file. Each report includes a data vintage table showing the exact refresh date for each source layer.
