Every data point in a CareIndex county report is directly traceable to a primary federal source. Derived metrics are labeled as CareIndex analysis with government inputs listed.

County boundaries are resolved using the Census Bureau ZCTA-to-county crosswalk (2010 vintage). Match rate: 99.6% of HHA ZIP codes resolve to a county FIPS. Agencies are attributed to counties based on their CMS-registered ZIP code.
